Purpose: Opportunity for the patient to ASK QUESTIONS Offer and SHARE INFORMATION about the medicines WITH THE PATIENT Establish whether the patient and health professional have SIMILAR OR DIFFERENT VIEWS about medicines CHECKS THE PATIENT’S READINESS, ability and intent in taking the medicines Ensure that patients know if SYMPTOMS CHANGE or problem persists
