Small bilateral parenchymal opacities with multinodular pattern or reticular pattern 15-20% normal chest, begins lower lung zone, bilateral plaques on parietal pleura Combined interstitial and pleural involvement may cause hazy ground-glass appearance to the CXR blur heart making a shaggy heart sign. Honeycombing may occur in advanced stages HRCT: up to 30% pt get abnormal HRCT even with normal CXR, findings include: subpleural densities, basilar dorsal fibrosis, coarse parenchymal bands, honeycombing , pleural plaques
