When do we suspect AIHA?

➢ AIHA should be suspected in any patient with a hemolytic anemia (as suggested by the presence of anemia and reticulocytosis). ➢ The peripheral smear usually shows microspherocytes (and the reticulocyte count will be high). ➢ Laboratory tests typically suggest extravascular hemolysis (eg, hemosiderinuria is absent; haptoglobin levels are near normal, schistocytes are absent on smear). ➢ Spherocytosis and a high mean corpuscular hemoglobin concentration (MCHC) are typical.
