NRTIs and lactic acidosis—NRTI agents, taken alone or in combination with other antiretroviral agents, may cause lactic acidemia and severe hepatomegaly with steatosis. Risk factors include obesity, prolonged treatment with NRTIs, and preexisting liver dysfunction. Consideration should be given to suspension of NRTI treatment in patients who develop elevated aminotransferase levels.
